Eligibility Documentation

Acceptable Proof of Veteran or Military Status

To verify eligibility, applicants should provide one of the preferred low-risk documents listed below whenever possible. Applicants may redact Social Security numbers, financial information, medical information, addresses, dependent information, and any other nonessential personal details before submission.

Please do not upload a photo or copy of a CAC, military ID, or TRICARE card. These documents may be shown for visual verification only. If submitting another document, please redact sensitive information that is not needed to confirm military or veteran eligibility.

Preferred low-risk proof

  • VA-issued Veteran ID Card
  • VA Healthcare ID Card
  • Driver's License or State ID with Veteran designation
  • State-issued Veteran Identification Card
  • VA Benefits Letter, with sensitive details redacted

Other acceptable proof

  • DD Form 214, with SSN and other sensitive information redacted
  • NGB Form 22, with SSN and other sensitive information redacted
  • Statement of Service Letter issued by a commanding officer, S1, personnel office, DMDC, or other authorized military representative
